Choline and Anxiety

Choline and Anxiety: What Does the Research Show?
A consistent pattern is emerging in modern life.
We are more stimulated, yet mentally flatter.
More connected, yet increasingly isolated.
More productive, yet struggling with:
Many people describe themselves as “wired but exhausted.”
Usually, the conversation stops at stress management.
Meditation. Breathwork. Sleep hygiene.
All useful.
But emerging neuroscience research into choline and anxiety suggests something deeper may also be happening.
Chronic anxiety and elevated arousal may alter the brain's metabolism of choline-containing compounds.
What Is the Relationship Between Choline and Anxiety?
A 2025 meta-analysis published in Molecular Psychiatry examined brain chemistry across several anxiety disorders using proton magnetic resonance spectroscopy.
Researchers analysed 25 datasets involving more than 700 participants with conditions including:
- generalised anxiety disorder
- panic disorder
- social anxiety disorder
One finding stood out:
People with anxiety disorders showed lower levels of choline-containing compounds in cortical brain regions.
These differences were particularly relevant to cortical areas involved in higher-order cognitive and emotional processing.
Importantly, this doesn't necessarily mean people with anxiety are simply “low in choline.”
The researchers discussed several possible explanations, including altered membrane phospholipid metabolism and increased neuro-metabolic demand associated with chronic arousal.
This raises an interesting possibility:
Anxiety may affect how choline-containing compounds are being used and recycled within the brain.
Why Choline Matters for Brain Health
Typically, people associate choline with:
- eggs
- brain health
- strength training
- pregnancy
- breastfeeding
But choline also plays several fundamental roles within the nervous system.
It is involved in:
- phospholipid membranes
- acetylcholine production
- methylation reactions
- neuronal signalling
- membrane structure and turnover
One particularly important compound is phosphatidylcholine, a major phospholipid found in cell membranes.
This matters because healthy neuronal membranes are fundamental to normal communication between brain cells.
Does Anxiety Deplete Choline?
We can't yet say that anxiety directly depletes dietary choline.
What the research shows is that people with anxiety disorders had lower concentrations of certain choline-containing compounds in cortical brain regions.
The researchers proposed that chronic arousal and stress-related neuromodulators such as norepinephrine could influence phospholipid metabolism, methylation pathways and the turnover of choline-containing compounds.
So the relationship between choline and anxiety may be more complex than simply being deficient in a nutrient.
Anxiety may change how the brain uses, metabolises or turns over choline-containing compounds.
Choline, Anxiety and Brain Fog
Anxiety and chronic stress can also occur alongside cognitive symptoms such as:
- mental fatigue
- difficulty concentrating
- forgetfulness
- reduced mental sharpness
- poor sleep
- feeling unable to switch off
This makes the relationship between choline, anxiety and brain fog particularly interesting.
Choline contributes to normal neurological function through its roles in cell membranes and neurotransmitter synthesis.
However, the 2025 study did not demonstrate that increasing choline intake treats anxiety or brain fog.
Rather, it provides evidence that anxiety disorders are associated with measurable differences in cortical choline-containing compounds.
Choline and Anxiety: Does Diet Matter?
Another relevant issue is dietary intake.
Many adults fail to meet recommended choline intakes, according to dietary surveys referenced in the research.
Important dietary sources of choline include foods such as eggs, meat, fish and dairy products.
This raises an interesting nutritional question:
If chronic stress alters the metabolism or demand for choline-containing compounds, could adequate dietary choline become particularly important during periods of prolonged mental stress?
It's a biologically plausible question, but one that requires more clinical research.
Sunflower Lecithin, Choline and Anxiety
Sunflower lecithin naturally contains phospholipids, including phosphatidylcholine.
This makes it an interesting dietary source of choline-containing phospholipids for people interested in brain and nervous system nutrition.
However, an important distinction needs to be made:
The anxiety study did not test sunflower lecithin or choline supplementation.
It therefore cannot tell us whether taking sunflower lecithin reduces anxiety.
What it does show is that choline-containing compounds and phospholipid metabolism appear to be altered in anxiety disorders, providing an interesting direction for future nutritional research.
Can Choline Help With Anxiety?
At present, there isn't enough evidence to say that choline supplements treat anxiety.
The stronger conclusion from the current research is that choline-containing compounds appear to be involved in the neurobiology of anxiety.
That distinction matters.
The 2025 meta-analysis found a consistent reduction in cortical choline-containing compounds across anxiety disorders, but intervention studies are needed to determine whether increasing dietary choline or supplementing with choline-containing compounds produces meaningful improvements.
For now, choline should be viewed as an essential nutrient involved in normal brain function, not as an anxiety treatment.
Choline and Anxiety: The Take-Home Message
The relationship between choline and anxiety is an emerging area of neuroscience.
Research has found lower cortical concentrations of choline-containing compounds in people with anxiety disorders and suggests that chronic arousal may influence phospholipid metabolism within the brain.
That doesn't mean anxiety is caused by low choline.
And it doesn't mean taking choline will cure anxiety.
But it does add another dimension to how we think about chronic stress.
Mental stress isn't purely psychological. It occurs within a metabolically active organ that depends on nutrients, neurotransmitters, cellular membranes and energy metabolism to function normally.
Understanding how choline and anxiety interact may eventually help us better understand the nutritional demands placed on the brain during prolonged periods of stress.
