Effects of Creatine Supplementation with and without Exercise and Diet Intervention

TL;DR A study of 64 adults found that taking 10 g of creatine daily without exercise increased lean tissue by over 1kg and improved strength. Combining creatine with exercise and diet produced broader improvements, while creatine was also associated with improvements in measures of memory and cognition.

Can creatine still provide meaningful benefits if you don't exercise?

A new 2026 study investigated exactly that, comparing creatine monohydrate supplementation with and without exercise and diet intervention in middle-aged and older adults.

The results were particularly interesting because creatine was associated with increases in lean tissue mass and strength even in participants who weren't following the structured exercise program.

As expected, combining creatine with exercise and dietary intervention produced broader improvements. 

How Was the Study Conducted?

The study included healthy, sedentary adults aged 45-65.

Participants first chose whether to participate in the structured exercise program.

Within those groups, they were randomly assigned in a double-blind manner to creatine monohydrate or placebo.

This produced four groups:

  1. No structured exercise + placebo
  2. No structured exercise + creatine
  3. Exercise/diet intervention + placebo
  4. Exercise/diet intervention + creatine

The exercise intervention involved resistance and aerobic exercise three times per week for 12 weeks.

Participants taking creatine received 5 g twice daily, for a total of 10 g per day.

Creatine Without Exercise: What Happened?

This was arguably the most interesting part of the study.

After 12 weeks, the no-exercise creatine group gained approximately 1.1 kg of lean tissue mass, compared with essentially no improvement in the placebo groups.

The researchers also reported improvements in strength and muscular endurance despite these participants not completing the structured training program.

Note: That doesn't mean creatine replaces exercise.

Rather, it suggests that creatine supplementation itself may have meaningful effects on lean tissue and physical function in middle-aged and older adults.

Creatine With Exercise and Diet

Exercise remained extremely important.

Participants completing the exercise and dietary intervention generally experienced greater reductions in weight and fat mass.

Adding creatine appeared to improve the outcome further.

The creatine + exercise group increased lean tissue mass by approximately 1.27 kg after 12 weeks, while the exercise + placebo group changed by only around 0.14 kg.

Body-fat percentage also fell more in the creatine + exercise group than in the other groups.

What About Creatine and Cognitive Function?

Researchers also performed an extensive battery of cognitive tests.

There were some encouraging findings involving delayed word recall, recognition, reaction time and measures of memory.

However, creatine did not improve every cognitive test, including several measures of attention and processing.

Importantly, the authors specifically describe the individual cognitive findings as exploratory.

So this study adds to the emerging research around creatine and cognition, but it shouldn't be interpreted as proof that creatine improves memory or cognitive function in everyone.

Practical Take-Home Messages

1. Creatine isn't only useful when you're training.
In this study, 10 g/day of creatine without structured exercise increased lean tissue mass and improved selected measures of strength and endurance.

2. Exercise still matters.
The broader body-composition improvements occurred when exercise, dietary intervention and creatine were combined.

3. Creatine may become increasingly relevant with age.
Preserving muscle mass and strength becomes progressively more important through middle and older age.

4. The dose used was higher than the typical 3–5 g/day maintenance dose.
Participants consumed 10 g/day, so the results shouldn't automatically be assumed to apply to lower doses.

5. The cognitive results need more research.
Some results were promising, but they were not consistent across all cognitive measures and were considered exploratory by the researchers.

The Bottom Line

The effects of creatine supplementation with and without exercise and diet intervention in this study suggest that creatine may provide benefits beyond simply enhancing resistance-training adaptations.

Perhaps most notably, middle-aged and older adults taking 10 g of creatine per day without structured exercise gained approximately 1.1 kg of lean tissue over 12 weeks.

But the study also reinforces an important point: the strongest overall strategy isn't creatine instead of exercise.

It's creatine alongside exercise and good nutrition.
